Position Description: Protingent Staffing has an exciting Direct Hire opportunity for Part time LPN (Assisted Living Facility) with our client that is located in Montebello, NY.
Job Responsibilities:
- Registered Professional Nurse who administers skilled nursing care to patients requiring professional nursing service in an assisted living facility.
- Calls the home care coordinator, RN or supervisor with verbal report of initial findings.
- In conjunction with the family and physician and/or authorized practitioner and the Supervisor adheres to the Plan of Care based on the RN’s assessment.
- Provides nursing care as outlined in the Plan of Treatment to include the following: Nursing services, treatments and preventive procedures including those requiring substantial specialized skill and ordered by the physician or authorized practitioner.
- Initiates preventive and rehabilitative nursing procedures as appropriate for the patient’s care and safety.
- Evaluates the effectiveness of his/her nursing service to the individual and family.
- Observes signs and symptoms and changes in the patient’s physical or emotional condition and consults with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the supervisor, as necessary.
- Provides and completes nutritional assessments under the direction of Director of Nutrition.
- Reports all patient changes and information to the Primary RN, Supervisor or Case Manager of home care on a timely basis.
- Submits timely records of visits and vouchers based on each visit
Job Qualifications:
- Must Have NYS LPN License.
- Must Have CHHA Experience preferred.
- Must Have Home care experience a plus.
- Must Have New grads welcome
Job Details:
- Job Type: Part time
- Schedule is Week1: Friday, Saturday, Sunday 7A-5P Week 2: Monday, Thursday, Friday 7A-5P
- Pay Range: $35/hour
- Location: Montebello, NY.
