Position Description: Protingent Staffing has an exciting direct hire opportunity for ACF RN with our client that is located in NY.
Job Description:
- The ACF RN is a Registered Professional Nurse who will manage all aspects of patient care related to services provided in the home. RN will have to travel to all of their ACFs.
- Candidate must drive.
Job Responsibilities:
- Conducts home care assessments and re-assessments using tools like the OASIS assessment tool as needed.
- Communicates initial findings to the home care coordinator or supervisor through a verbal report.
- Collaborates with family, physicians, authorized practitioners, and the client's Supervisor to develop and implement a Plan of Care.
- Provides skilled nursing care as per the Plan of Treatment, including specialized procedures ordered by the physician or authorized practitioner.
- Initiates preventive and rehabilitative nursing procedures for patient care and safety.
- Evaluates the effectiveness of nursing services provided to patients and their families.
- Monitors and reports signs, symptoms, and changes in the patient’s physical or emotional condition to the physician or supervisor.
- Updates the supervisor or home care coordinator promptly about patient changes or relevant information.
- Submits daily records of visits and hours worked.
- Participates in case management conferences with the care team.
- Educates, supervises, and counsels patients and caregivers regarding nursing care and related concerns.
- Documents care provided, patient responses, and changes in condition as per the client's policies.
- Ensures all documentation aligns with the client's policies and procedures.
- Attends in-service training and continuing education programs.
- Participates in Performance Improvement activities.
- Supervises and orients home health aide staff, documenting supervision as per the client's policies.
- Adheres to client rights, confidentiality policies, infection control, and standard precautions.
- Observes and respects Advance Directives as applicable.
- Reports client changes to the supervisor as needed.
- Completes statistical data when assigned to an adult care facility.
- Coordinates total patient care in collaboration with the home care coordinator or supervisor if assigned full-time to adult care facilities.
- Responds professionally and promptly to emergencies in adult care facilities when assigned.
- Participates in all other duties as assigned.
Job Qualifications:
- NYS RN License
- Must have own vehicle to travel to patients.
- CHHA experience is a plus
